In order to download and print the brochure and application, you must have Adobe Acrobat reader installed on your computer. If you do not have Acrobat reader, you can download it for free by clicking on the 'Get Acrobat Reader' button below. 

The Denta-Chek application can be completed on your computer, but you will need to print out the application, sign and mail it along with your initial payment to:
